Special Diet Statement
Please fill out the Special Diet Statement form if your child has any special dietary requests. This form must be completed by a licensed physician, physician assistant, or registered nurse. Updates to this form are required only when a participant’s needs changes.

This form is for those wanting to allow their child to get a second entree at lunch time.
Please fill out this form for new students and/or families who with to make a change to a prior signed agreement. This agreement will remain in place until withdrawn by the parent/guardian.
